Baptismal, confirmation, marriage and death certificates are issued from the parish register. Fill this in and the office will search for the record, then tell you the fee and when it is ready to collect.
As it appears in the register — the maiden name on a marriage record, for example.
The year the sacrament was celebrated.

Registrars usually ask, and some documents are annotated differently depending on the reason.
